Abstract
This study was carried out to compare two anti-Helicobacter Pylori regimens in patients referred to endoscopic section of Kashani Hospital mainly due to hematemesis, melena, intractable and long-term epigastric pain with peptic ulcer in their endoscopy. The exclusion criteria were negative urease test and negative Helicobacter Pylori (H.pylori) in biopsy sample with Giemsa stain. Based on the exclusion criteria 14 cases out of 61 patients in the first endoscopy have been excluded from the study. The patients were between 14-80 years old and the male to female ratio was 1.1/2. Also 21.3% of them had familial history of peptic ulcer (PU). The patients were divided into two groups, A (26 cases) and B (21 cases). The regimen of group A consisted of omeprazol 20 mg BID+Amoxicillin 500 mg QID+Metronidazol 250 mg TDS daily for two weeks and regimen of group B consisted of Bismuth 120 mg BID+Tetracycline 250 mg QID+Metronidazol 250 mg TDS daily for two weeks. One month after the last treatment, they were invited for the second endoscopy and checked for positive H.pylori and ulcer healing. 17 cases of group A and 18 cases of group B returned for the second endoscopy. One patient of group A and three patients of group B were excluded based on the exclusion criteria. H.pylori had been eradicated in 67.7% of group A and in 60% of group B, also healing rate was 75% in A and 66.6% in B. The differences between two groups were not significant. The small insignificant higher H.pylori-eradication and healing rate in group A might favors the use of regimen A.
